This session explores the gap between what clinicians assume patients understand and the realities of patient and caregiver comprehension, highlighting how social determinants of health, care transitions, and caregiver variability contribute to these discrepancies despite standard education efforts. Attendees will review practical strategies to assess health literacy and explore emerging opportunities to leverage technology and personalized education to identify risk and improve patient safety in home infusion care.

With over 14 years of experience in critical care, home infusion, and clinical education, her work focuses on home parenteral nutrition, vascular access safety, and patient-centered education. Elena also works closely with patients and care teams to address day-to-day challenges in managing complex home infusion therapies.

Speakers will review key components of onboarding and ongoing management of patients on complex infusion therapies, including home parenteral nutrition (HPN). The session will also address how clinicians can remotely address common complications, infusion pump issues, troubleshooting strategies and the importance of collaboration between disciplines.

As National Director of Pharmacy Operations at Soleo Health, she drives operational efficiencies, supports regulatory compliance, advances performance improvement initiatives, and partners across teams to deliver safe, efficient, patient-centered care. Bridget earned her BS in Pharmacy degree at the University of Rhode Island and then went on to complete a residency at Children’s National Medical Center, Washington DC, where she focused on neonatal TPN in the NICU. Her 34 year career spans retail, hospital, home infusion and specialty pharmacy with a strong focus on quality patient care.

Putting the Pieces of the Puzzle Together – Understanding PN Compatibility and Stability

Parenteral nutrition (PN) is a high-alert medication used across all care settings. One of the greatest risks to safe PN practices today is the lack of clinician education focused on PN compatibility and stability principles. Of all clinicians involved in nutrition support (physicians, dietitians, pharmacists, nurses, advanced practice providers), pharmacists receive the most education and training on medication compatibility and stability, but even pharmacy school curricula have minimized student and trainee exposure to this content. Many pharmacists entering the profession have limited knowledge of PN compatibility and stability and rely on summaries in online tertiary resources (which are easily misinterpreted), simplistic charts from product manufacturers, or institution-specific policies, many of which are likely outdated and created based on PN products that are no longer commercially available. Newer data have demonstrated that PN products (e.g., 15% amino acid solutions; lipid injectable emulsion products) are not interchangeable in terms of compatibility and stability. With the introduction of many new PN products to the market, there is an increased need for reappraisal of PN compatibility and stability data and practices. The regression of nutrition support expertise among healthcare professionals has occurred simultaneously with the expanded use of PN in different clinical scenarios. All disciplines within nutrition support need practical resources to assist with interpretation of PN compatibility and stability data and incorporation of this data into institutional practice. This presentation will focus on the principles of PN compatibility and stability, testing methods used for evaluating PN compatibility and stability, and practical ways for nutrition support professionals of all disciplines to incorporate PN compatibility and stability safeguards into their daily clinical practice.
Gordon S. Sacks, PharmD, BCNSP, FASPEN, FCCP

Dr. Sacks earned a Doctorate of Pharmacy degree from the University of Texas (1994) after receiving his BS degree in Pharmacy from Auburn University (1989). His long academic career included roles as Associate Professor of Pharmacy Practice (University of Mississippi), Clinical Professor and Chair of Pharmacy Practice (University of Wisconsin – Madison), and Professor and Department Head of Pharmacy Practice (Auburn University). During this time, he also maintained an active clinical role serving as Coordinator of the Nutrition Support Team at all 3 academic medical centers. In July 2018, Dr. Sacks joined Fresenius Kabi, USA, as Senior Director of Medical Affairs for the Parenteral Nutrition Market Unit and remained in this position until April 2024. Dr. Sacks returned to the direct patient care arena in July 2025 to help establish the Home Parenteral Nutrition Service for intestinal failure patients at UNC Medical Center – Chapel Hill, North Carolina. He has been a board-certified Nutrition Support Pharmacist since 1995. He previously served as the President of the American Society for Parenteral and Enteral Nutrition (ASPEN) from July 2015 – June 2016.

She specializes in building sustainable home enteral and parenteral nutrition programs, optimizing revenue cycle processes, and guiding home infusion providers across the country toward stronger clinical, operational, and financial outcomes to provide exceptional patient care.

Jacquelyn Oberman is a Clinical Nutrition Program Manager leading Nutrition Reimbursement and Medicare at Optum Infusion Pharmacy. After graduating from the University of Memphis with a Masters degree in Clinical Nutrition, she found her love for nutrition support working in the pediatric intensive care unit. Jacquelyn has spent the last 10 years at Optum and has almost 15 years of experience in home infusion. With significant interest in consumer advocacy and sustainable access to infusion therapy, her recent speaking appointments have addressed financial implications of health equity, unconscious bias in home infusion, and barriers to coverage for home parenteral nutrition (PN).
