To support this goal, the American Society for Parenteral and Enteral Nutrition (ASPEN) has developed two practical, easy-to-use checklists designed to assist both patients and healthcare providers.

Patient Discharge Checklist
This checklist empowers patients and caregivers to understand their tube feeding regimen. It provides guidance on administration techniques and clarifies who will manage care and supply delivery at home. By prompting important questions and documentation, it helps build confidence and competence in managing enteral nutrition independently.

Clinician Discharge Checklist
This tool helps healthcare teams navigate the critical steps before discharge, including assessing feeding tolerance, selecting appropriate formulas, addressing hydration needs, and coordinating referrals to home nutrition suppliers with insurance authorization. It also emphasizes the importance of thorough patient education and hands-on training to ensure readiness for home care.
